How did symptom scoring change benzodiazepine use in a hospital withdrawal program?

A medication chart may show a benzodiazepine given only after certain symptoms were documented. A hospital study from South Africa illustrates how a scoring protocol can change what appears in that chart.

What the hospital observed

Muddapah and Weich compared 135 admissions before and after implementation of the CIWA-Ar alcohol withdrawal scale at Stikland Hospital. In the later group, 33.8% received benzodiazepines compared with 51.4% in the earlier group. The median total went from 5 mg to 0 mg. Neither group had a recorded withdrawal complication in the study.

Why the chart matters more than the cohort average

The groups came from different years, and 135 admissions provide limited information about uncommon complications. To reconstruct one admission, read the dated symptom scores alongside the medication administration record and any clinician notes explaining deviations. A group median of zero can coexist with treatment for individual patients who met the protocol’s criteria.

What Does a Similar EEG Frequency Tell Us About Midazolam and Dup15q?

Two groups can show a similar EEG frequency for different reasons. Researchers can use that resemblance to test a biological hypothesis, while an individual EEG still needs clinical context. How the study made the comparison Frohlich and colleagues compared EEG recordings from children with Dup15q syndrome with recordings from typically…
