What does a prescribing alert establish in an electronic medical record?
A medication alert in an electronic record is one step in the prescribing process. The entry is easier to interpret when it can be tied to the underlying trigger and the clinician’s response.
What the alert study tracked
A multicenter observational study followed the staged introduction of a clinical decision support program for opioid and benzodiazepine orders. It reported 555,626 alerted encounters and classified 100,301, about 18%, as encounters in which the alert influenced prescribing. The unit was an encounter. One patient could appear more than once, and the analysis measured recorded prescribing responses rather than subsequent overdoses.
Which records answer the next question
When reconstructing an individual encounter, distinguish the proposed order, the alert, any override or cancellation, and the final order. Then check the dispensing or administration record for evidence that the medication reached the patient. Even when an alert and an adverse event appear in the same chart, the event’s cause depends on the exposure timeline and clinical evidence.
