The Imaging Baseline Was a Visual Control Condition
In the fMRI task, “baseline” referred to viewing blank blue rectangles. It was not a scan showing each participant’s untreated brain activity before CBD.
The contrast was within the task
During encoding, participants viewed word pairs; during recall, they supplied a previously associated word. During the baseline condition they viewed two blank rectangles of matching dimensions. BOLD responses during encoding and recall were contrasted with that visual control condition to estimate task-related activation.
A contrast is not an absolute activity measurement
The resulting signal describes a difference between modeled conditions. It does not directly quantify neurotransmitter concentration, tissue injury or every aspect of resting brain function. Changing the task or reference condition could ask a different scientific question. The experiment also compared different participants assigned to CBD and placebo, rather than scanning everyone under both treatments.
Avoid an individual normalization claim
Selected regional activity in the CBD group was intermediate between placebo and healthy comparators, but task performance did not significantly improve. The visual baseline cannot be used as proof that an individual’s previously abnormal activation became normal after treatment. The study supplies acute mechanistic evidence with a defined contrast, leaving sustained clinical benefit and psychosis prevention unresolved.
