CBD Powder Versus Oil: A Higher Trend Is Not a Significant Parent-Drug Difference
A crossover trial found faster absorption with a CBD powder formulation, but a trend toward greater parent-CBD exposure under fasting conditions should not be described as a proven significant increase. Parent drug, metabolites and composite exposure measures gave different answers.
Four conditions within participants
Healthy adults received 400 mg CBD as CBtru powder and as Epidyolex oil under both fed and fasted conditions. The randomized, open-label crossover design included at least a 14-day washout between administrations.
Comparing conditions within the same participant helps address stable person-level differences. It does not eliminate meal effects, period effects, sampling limitations or the distinction between pharmacokinetics and clinical outcomes.
Do not combine unlike endpoints
Under fasting conditions, parent-CBD exposure and peak concentration trended higher with the powder. Metabolite exposure and the authors’ combined drug-exposure measures showed significant increases. A sum that includes metabolites is not identical to the measured parent-CBD concentration.
With food, exposure increased for both products, while the paper described their exposure profiles as comparable. The powder reached its median peak earlier under both meal conditions. Earlier absorption does not by itself prove superior treatment benefit or greater impairment.
Commercial and clinical context
The authors disclosed funding and employment relationships involving the product developer and the contracted research organization. These interests are relevant context for a formulation-development study.
The experiment used single doses in healthy participants. It did not establish improved disease control, equal therapeutic efficacy or safety in every patient population. A careful comparison names the endpoint, statistical conclusion and meal state rather than collapsing all results into ‘better bioavailability.’ For a disputed result, formulation and timing belong in the reconstruction.
