CBD Metabolite Ratios Could Fall While Metabolite Exposure Increased

A lower metabolite-to-parent ratio did not necessarily mean less metabolite exposure in the CBD food study. The parent CBD exposure could increase more strongly, changing the ratio’s denominator.

Absolute exposure and relative exposure answer different questions

The trial measured CBD, 7-hydroxy-CBD and 7-carboxy-CBD in plasma after a single 750 mg pharmaceutical CBD dose. Researchers calculated exposure for each analyte and ratios comparing metabolite exposure with parent-drug exposure.

After the high-fat, high-calorie meal, 7-hydroxy-CBD exposure increased relative to fasting. Yet its reported metabolite-to-parent ratio for area under the curve to infinity was lower: approximately 0.426 in that meal condition versus 0.876 while fasting.

The denominator explains the apparent conflict

There is no contradiction in an increasing numerator accompanying a declining ratio if the denominator rises more. A statement that food “reduced the metabolite” would therefore be incomplete unless it named whether it meant absolute concentration, exposure or the ratio to CBD.

The authors discussed metabolite ratios as potentially reflecting absorption or biotransformation effects. The ratio alone does not isolate one enzyme pathway or establish which mechanism caused the change.

Implications for interpreting a plasma result

Name both analytes, the exposure metric and the sampling context before using a ratio. These were group pharmacokinetic findings in healthy adults, with substantial variability and a specific oral formulation. They did not establish a metabolite ratio that diagnoses clinical response, impairment or a precise time of use. The study received support from the formulation’s manufacturer.

The CBD Meal Comparison Changed Calories and Composition Together

The CBD trial compared complete meal conditions, not an isolated change in dietary fat. Its high-fat, high-calorie and low-fat, lower-calorie breakfasts differed in several respects at once. What the meal labels contained The high-fat, high-calorie breakfast supplied approximately 918 kcal, with a similar-calorie vegetarian substitution available. The low-fat, lower-calorie breakfast…
