What the Reported CBD Food-Effect Multiples Represented

The published CBD food-effect multiples were modeled ratios of geometric least-squares means. They did not mean that every participant’s concentration rose by the same multiple after a meal.

The analysis compared relative exposure

Healthy adults received a single 750 mg dose of a pharmaceutical CBD oral solution under assigned fasting, meal or drink conditions. Plasma sampling continued for up to ninety-six hours.

For exposure measures such as area under the concentration–time curve and peak concentration, the investigators used log-transformed data in a mixed-effects analysis. They reported test-to-reference ratios of geometric least-squares means with 90% confidence intervals.

A group ratio is not an individual prediction

The high-fat, high-calorie comparison produced approximately 3.8-fold greater area under the curve to infinity and 5.2-fold greater peak CBD concentration than fasting. These are different endpoints derived from a group analysis, not universal multipliers for every person.

The confidence intervals concern uncertainty in the estimated relative exposure. They are not prediction intervals specifying where every new patient’s result will fall. The authors reported moderate to high between- and within-person variability, and the exploratory trial had no prospective power calculation.

What the numbers cannot supply

A ratio of plasma exposures does not establish a matching ratio of pain relief, seizure benefit or impairment. It also cannot reliably reconstruct an individual’s dose from one sample without the relevant formulation, food history and timing. The sponsor-supported study tested a particular high-dose oral solution in healthy adults rather than every retail CBD product.

The CBD Meal Comparison Changed Calories and Composition Together

The CBD trial compared complete meal conditions, not an isolated change in dietary fat. Its high-fat, high-calorie and low-fat, lower-calorie breakfasts differed in several respects at once. What the meal labels contained The high-fat, high-calorie breakfast supplied approximately 918 kcal, with a similar-calorie vegetarian substitution available. The low-fat, lower-calorie breakfast…

Why Lower Apparent CBD Clearance After Food Does Not Isolate Elimination

The food study’s lower apparent oral clearance cannot be read as proof that the body eliminated CBD more slowly. The reported quantity includes bioavailability, which also changed with the dosing condition. The parameter was CL/F Researchers gave healthy adults a single 750 mg dose of a pharmaceutical CBD oral solution…

CBD Metabolite Ratios Could Fall While Metabolite Exposure Increased

A lower metabolite-to-parent ratio did not necessarily mean less metabolite exposure in the CBD food study. The parent CBD exposure could increase more strongly, changing the ratio’s denominator. Absolute exposure and relative exposure answer different questions The trial measured CBD, 7-hydroxy-CBD and 7-carboxy-CBD in plasma after a single 750 mg…
