A Brownie Was Reportedly Made With CBD Oil. What Did the Urine THC Screen Add?

A 2023 case report described acute confusion in a 94-year-old patient. During admission, a relative disclosed that the patient had been given brownies prepared with an oil marketed as pure CBD. Clinicians then obtained a urine drug screen reported as positive for THC, and the patient returned to baseline with supportive care.

Two sources of evidence, two different questions

The relative’s account identifies a possible product and route of exposure. The urine result adds laboratory evidence consistent with cannabinoid exposure, subject to the scope and limitations of the screening method. The report describes a urine screen. It does not present direct chemical testing of the oil or brownie.

What would connect the product to the result?

Preserve the actual container, lot information, and any remaining material. Review the screen target, cutoff, and whether a specific cannabinoid or metabolite was confirmed. That work helps separate exposure, product attribution, and the clinical cause of symptoms. The case illustrates why all three questions can arise even when the reported label sounds unambiguous.
