When Low Sodium and Confusion Follow CBD Use During Sertraline Treatment
When confusion begins after a product change, a medication timeline and objective laboratory measurements can help organize competing explanations. A published case involved an individual who added CBD after years on sertraline and later developed low serum sodium and cognitive symptoms.
The observation and the proposed mechanism
The reported CYP2C19 genotype was consistent with intermediate metabolism. The authors proposed that CBD further inhibited this enzyme, increasing sertraline exposure and contributing to hyponatremia. The sodium value reached 120 mmol/L, and the authors reported that cognition improved as sodium rose. The sodium measurement and clinical course are direct observations. The change in sertraline exposure was the authors’ proposed explanation.
What to establish in a case review
Document the start and stop dates, formulation, dose, sodium values and symptom onset. Consider other medications, illnesses and changes in fluid balance that could account for low sodium. A genotype result can inform a mechanism, while the broader clinical timeline determines how well that mechanism fits an individual record.
