Pain Relief and Pain Freedom Measure Different Migraine Outcomes

When a migraine study reports that pain improved, the endpoint determines what that claim means. In a randomized crossover study, Schuster and colleagues had participants treat separate migraine attacks using different vaporized formulations or placebo. The version examined here is the 2024 preprint.

Two questions at the same time point

At two hours, 67.2% of attacks treated with the combined THC/CBD formulation met the pain-relief endpoint, compared with 46.6% with placebo. The pain-freedom figures were 34.5% and 15.5%, respectively. Both outcomes were assessed at the same time, but pain relief allowed a degree of pain to remain.

The study randomized 92 participants and recorded 247 treated attacks. Because individuals could treat more than one attack, the attack count describes repeated observations within participants. When citing the trial, specify both the endpoint and the assessment time. A two-hour result for a particular formulation answers a narrower question than a claim about preventing future migraines.
